Delta Air Lines Flight 1141 was a regularly scheduled domestic passenger flight from Dallas/Fort Worth International Airport in Texas to Salt Lake City International Airport in Utah. On August 31, 1988, the Boeing 727-200 operating the flight crashed during takeoff, resulting in the deaths of 14 of the 108 people on board.
The National Transportation Safety Board (NTSB) determined that the cause of the crash was due to the flight crew's failure to set the flaps and slats to the proper takeoff configuration before attempting to take off. This error resulted in the aircraft being unable to achieve the necessary lift to become airborne. Contributing factors to the crash included crew fatigue, lack of crew coordination, and inadequate crew training.
